Advance Care Planning: Let's Start the Discussion
Deb Bowman, Lyngblomsten Community Wellness Nurse, will give you ideas on how to get the discussion started about advance care planning (ACP): a guided conversation about end-of-life care wishes and preferences.
This conversation should take place between loved ones and with people that you want and trust to speak for you. There is no right or wrong in ACP, just letting others know your decisions and values.
We will also discuss completing a Health Care Directive and naming a Health Care Agent. Copies of Honoring Choices Minnesota will be the main resource available for writing your Health Care Directive.
If you want individual help with filling out the Honoring Choices document, please register for private discussion with a counselor after the presentation (3–4 PM).
