Diabetes Prevention Program

Simply follow these two steps:

1)  Attend a FREE information session and screening to see if you qualify for thi Diabetes Prevention Program. Screening includes:  diabetes risk assessment, blookd work, body mass index, nutrition consultation, and exercise demonstration.

Why attend this open house? If you have the risk factors of a family history of diabetes, high blood pressure or high cholesterol, diabetes during pregnancy, or are overweight and/or inactive, this program may be for you!

Information & Screening Session  |  Monday, September 14
9:30–11:30 AM at the 5-5-1 Club (lower level of the Lyngblomsten campus at 1415 Almond Ave, St. Paul) 

2)  If your screening shows you qualify for the program, you will be able to register and participate. You will work with a coach who will help you learn the tools for leading a healthier lifestyle.

16-week Diabetes Prevention Program (after qualifying)
Mondays, September 28–January 11
9:30–10:30 AM at the 5-5-1 Club (lower level of the Lyngblomsten campus at 1415 Almond Ave, St. Paul)
