The Lokensgard at Lyngblomsten is the transitional care unit/rehabilitation neighborhood within the Care Center.  Lyngblomsten is proud of the continuum of care that we are able to offer.  As part of our ongoing interest in meeting the needs of seniors and others in the community, we have extended our range of services to include short-term health care and a place of recuperation for people following surgery or serious illness.

Patients who come to this area must meet the admission criteria established.  Once their condition has improved, they will either be discharged back to their homes or moved to another neighborhood or service house or apartment on our campus, depending on their original discharge plans.  Comfort and healing during the stay with us is our number-one goal.  We are committed to providing the best care possible, just as Lyngblomsten has done since 1906.

Complex nursing care such as, but not limited to:

  • Rehab:  physical, occupational, and speech  therapies (up to 7 days a week as ordered)
  • Post-surgical care
  • Cardiac rehab
  • IV therapy:  peripheral lines, central lines
  • Wound care, VAC therapy
  • Hemodialysis
  • Pain management
  • Diabetes management
  • Traction
  • Discharge planning
  • Ostomy care

Rooms in The Lokensgard include an electric bed, reading light, call light, nightstand, phone, chair, and TV with cable and DVD.  Patients are encouraged to bring personal items that will make the room as comfortable as possible.

The Lokensgard has a fully equipped kitchen and dining room used exclusively for our transitional care patients.

Admission Criteria

  1. Patient has a skilled nursing need that  requires frequent monitoring and/or receives therapies according to Medicare/HMO guidelines.
  2. Patient’s condition allows for reasonable expectation of discharge.
  3. Patient and patient’s support system demonstrate a willingness to actively participate in the plan of care.
  4. Patient must not demonstrate disruptive behavior due to cognitive impairments or mental illness that would infringe on other patients’ stays.

Discharge Criteria

Patients will be ready for discharge or transfer when they no longer meet any one of the admission criteria.
