What is short-term transitional care?
Short-term transitional care is a recuperative place to heal between hospital and home.
Our transitional care program has been around since 2005 to provide specialized rehabilitative care to individuals following a stay in a hospital. Working alongside you and your loved ones, our highly trained TCU team is experienced in creating a care plan which, from the very first day you arrive, helps prepare you for discharge back to home.
Patients who come to the TCU must meet the admission criteria established. Once their condition has improved, they will either be discharged back to their homes or more appropriate placement. They will also have the option to stay long-term on our campus.
